Mark will ensure that you are connected to the FH Clinical Pain Lead (Brenda Poulton) and the RAC for LBP Coordinator (Gerry O'Hanley) as required.
Referral into the program is only available to patients whose Primary Care Provider has enrolled. Please check with your Primary Care Provider.
