As of Friday, November 22,2024, the FH RAC-LBP Pilot will no longer be able to receive patient referrals. We are at capacity within the pilot and need to ensure completion of the patient Follow-up Visits by the Pilot closure on Friday, March 14, 2025.
We are currently working to secure sustainability funding and will communicate the outcome of this work as soon as possible.

If your patient does not have their own email address, please ask them to provide a family member or friend's email who can assist them. Incomplete referrals will not be processed and RAC LBP Admin will not contact patient's directly for missing information.**
