The RAC for LBP Pilot Registration and Onboarding Module provides an overview of referral criteria, the RAC for LBP Model of Care, and the important role primary care plays in managing low back pain patients. The module takes approximately 15-20 minutes to complete. Completion of this module is required to refer patients.
To refer your patients to the Pilot, you must be a Primary Care Provider (FP or NP) in the Fraser Health Region. If you have already registered with the Pilot, please email the RAC for LBP Pilot Office RACLBP@initiumcpm.com to obtain a referral form.
Once submitted, you will be contacted in 1-2 days with confirmation of registration.
